Locate the evaluation form below the video window. Answer membership enrollment questions, evaluate the activity, and create your certificate of completion with contact hour (CH) credit. Use the scroll bar on the right side of the form to view all questions.
At the end of the form, click “Submit”.
To print a certificate* for your personal records, click the three dots (...) icon in the upper-right corner of the form and select "Print Response."
