• Locate the YouTube video window, click the Play button, and watch the 60-minute recording.
• Locate the evaluation form below the video window. It will take 5 minutes to answer questions about your membership enrollment, evaluate the activity, and create your certificate of completion with contact hour (CH) credit. Use the scroll bar on the right side of the form to view all questions.
• At the end of the form, click “Submit”.
